The state's multicounty grand jury issued an indictment last week from its investigation of the Oklahoma Department of Veterans Affairs.
The defendant's identity was not disclosed because the indictment was sealed.
The grand jury began investigating the agency after veteran Jay Minter, 85, died last year when he was scalded in a whirlpool at the Claremore Veterans Center.
